Welcome to Comprehensive Eye, we cannot wait to see you! Please print, fill out, and bring the required documents with you to your appointment. Doing this will save you time during the day of your visit, so we encourage you to review these forms before visiting us and to let us know if you have any questions.

If you are a new patient, please print out and complete the New Patient Forms along with any other forms that pertain to your visit.

If you plan on using Medicare insurance, please print out and complete the ABN Medicare form, even if you are an established patient.

If you would like a contact lens prescription at your time of visit, please print out and complete the Annual Contact Lens Agreement, even if you are an established patient.

Late Policy: 

Patients are asked to arrive to their appointments before their scheduled appointment time. New patients are to arrive 15 minutes before their scheduled appointment time. Established patients are to arrive 5 minutes before their scheduled appointment time. This allows enough time for the registration process to be completed before the actual appointment time.

A grace period of 10 minutes will be permitted for unforeseen delays a patient may encounter while traveling to the clinic location for their appointment. If a patient arrives more than 10 minutes late for their appointment, the patient will be given the option of either being seen that day as a walk-in, if the schedule permits, or rescheduled for a later date.
